NEW PATIENTS

If you are interested in seeing one of our practitioners please send us a referral. We require a referral for triage before we can offer an appointment. Please ensure your referral contains your best contact number as we will try and contact you to arrange an appointment time.

Once you have an appointment scheduled you will be provided a new patient information pack. We request that you try and complete the forms ahead of time and send them back to us prior to your appointment. If you are unable to do so please notify our reception staff and arrive 15 minutes early to your appointment.

Consultation fees vary depending on the length of the consult and the practitioner you are seeing, please speak with reception for more information. Medicare rebates are available and we can arrange for these to be submitted on your behalf.

On the day of your new patient appointment please bring with you:

  • Referral letter, inclusive of your medical history and current medication regime.

  • Any relevant scans (film) or other investigations.

  • Medicare card, health insurance card, pension card or DVA card if applicable.

  • Complete information pack, if you are unable to send it in prior.

  • A copy of your enduring power of attorney if you have one.
