All of our practitioners are currently accepting new patients.

 

Our practice requires a referral for triage before we can book a new patient appointment. To ensure prompt triage, we require the following information to be included in the patient’s referral:

 

Format

• Referral on letterhead

• Doctor’s provider number

• Complete contact details for the patient

• Medicare number

• Private health number (if any)

 

Clinical Information

• Condition in which you are referring

• Complete Medical History

• Current Medications

• Copy of any MRI, CT and or pathology reports
